Who we are

This website is operated by Kelebogile Medical Centre, a family medicine practice in Pretoria. The practice is run by Dr Tebogo Setlogelo (MBChB (Medunsa), Dip. O&G (CMSA)).

What information we collect

The information we collect through this website is limited to the details you choose to give us. Specifically:

• Appointment requests: when you complete the appointment form, we collect your name, your phone number or email address, your preferred date and time, and any short note you choose to add about your reason for visiting. We ask you not to include clinical details in this form.
• Newsletter subscriptions: if you sign up for our monthly wellness letter, we collect your email address.
• Map interactions: the practice location map on our contact page is embedded from Google Maps. Google may set its own cookies when the map loads. This is governed by Google's privacy policy, not ours.
• Web analytics: at present this site does not run any analytics or tracking scripts. If we add analytics in future, this policy will be updated and the change will be disclosed clearly.

Clinical information about your health is collected in person during a consultation and is stored in your medical record at the practice. It is never collected through this website.

Why we collect it

We collect the information above for clearly limited purposes: to schedule and confirm your appointment, to deliver the wellness newsletter you have asked for, and to respond to any enquiry you send us. We do not use your details for any other purpose, and we never sell or share them with third parties for marketing.

How we store and protect it

The website itself does not store any of the information you submit. Form submissions are delivered to our practice email through Web3Forms, which transmits the data over an encrypted connection. The website is hosted on Cloudflare, which provides edge security and DDoS protection. Once received, appointment and enquiry details are kept only for as long as is needed to attend to your request, and clinical record-keeping is governed separately by HPCSA guidelines and the Records Retention requirements that apply to medical practices in South Africa.

Third-party processors

The website relies on a small number of third-party services:

• Cloudflare — hosts the website and serves it to visitors. Cloudflare may log standard request data such as IP addresses for security purposes.
• Web3Forms — relays appointment and newsletter form submissions to our practice email inbox.
• Google Maps — provides the embedded map on our contact page. Google sets its own cookies for this service.

Your rights under POPIA

Under the Protection of Personal Information Act, you have the right to:

• Request access to the personal information we hold about you.
• Ask for it to be corrected if it is inaccurate or incomplete.
• Ask for it to be deleted, where there is no overriding clinical or legal reason to retain it.
• Withdraw your consent to processing at any time, including unsubscribing from the newsletter.
• Lodge a complaint with the Information Regulator if you believe your rights have been infringed. The Regulator can be contacted at info@inforegulator.org.za.

To exercise any of these rights, please contact us using the details at the bottom of this page and we will respond within a reasonable time.

Cookies

The website itself does not set cookies. The Google Maps embed on our contact page sets cookies controlled by Google. If you would prefer not to load the map, you can use the address details listed alongside it instead.

Newsletter unsubscribe

Every newsletter we send includes a clear unsubscribe link at the foot of the email. You can also email us at hello@kelebogilemedicalcentre.org to be removed from the list. We will action the request promptly.

Changes to this policy

If we update this policy, we will change the "Last updated" date at the top of the page and, where the change is significant, we will draw attention to it on the home page for a reasonable period.
